To begin a new business relationship with Illinois Casualty Company, please download and complete and submit our Agency Profile, which is attached as a PDF file. In addition to the Agency Profile, the following additional documentation/information must be attached.

  1. Copies of loss runs from your commercial lines companies verifying your premium volume and loss ratios for the last three (3) years;
  2. A detailed plan of how you will meet your premium volume commitment with Illinois Casualty Company;
  3. Copy of your current E&O Declarations;
  4. Current copies of agency license and all producer licenses;
  5. Copies of financial statements for agency (last fiscal year and subsequent interim); AND
  6. Any additional agency materials (brochures, etc.) you feel have relevance to this decision.
